Continuous Care is to be provided only during periods of crisis to maintain a patient in their place of residence. The patient must require primary nursing care for palliation or management of actual medical symptoms to be eligible to receive continuous care.

Continuous Care cannot be provided in a skilled nursing facility (SNF), inpatient hospital or inpatient hospice facility.
A period of crisis is a period of time when the patient requires the higher level of “continuous care” for at least 8 hours in a 24-hour period (midnight to midnight) to achieve palliation or management of acute medical symptoms.
The care does not need to be “continuous” but must total 8 hours or more of care within 24 hour period.
The care must predominantly nurse care provided by RN or LVN. Homemaker of Hospice aide services may be provided to supplement nursing care. This means at least 50 percent of the total care provided must be provided by a nurse.

GIP is available to all hospice patients who are in need of pain control or symptom management that cannot be provided in any other setting. Skilled Nursing Care may be needed by a patient whose home support has broken down, if this breakdown makes it no longer feasible to furnish needed care in the home setting.

Hospice regulations require that the patient be at the facility’s highest level of care with a Registered Nurse available on site, 24 hours a day, 7 days a week. The facility is reimbursed by Hospice at a per diem rate for R&B and nursing care.

It is reimbursed for no more than 5 consecutive days per respite period. This care is provided on an occasional basis.
At the end of 5 days, the patient either returns home or remains as a resident in the care center, if accepted. The basic charges for room and board will then be covered by Medi-cal, Private Insurance or Private Pay.
